Our promise
Plain-English explanations for complex care and planning conversations.
A clear path through care settings, providers, insurance, and next steps.
A steady presence for your loved one and your family’s priorities.
Practical support that keeps people, information, and care plans aligned.
How we help
We bring practical structure to care decisions and family communication—without stepping into the role of a law firm or clinical provider.
We gather the care needs, family concerns, living arrangements, benefits questions, and existing information competing for your attention.
You leave with focused priorities, useful questions, and a written plan for conversations with providers, facilities, insurers, and trusted counsel.
For families who need ongoing support, we coordinate services, attend care conferences, and advocate so important follow-through does not fall through the cracks.
A focused starting point
A future paid session for families who need a clear, shared plan before the next call, visit, or decision. We'll confirm fit, scope, and availability before any engagement begins.
Roadmap consultations and family planning sessions are offered virtually or by phone. In-person support is reserved for facility visits and care-plan meetings.
01Clarify the urgent and important decisions
02Prepare for provider, facility, insurance, or attorney conversations
03Receive a practical next-step plan your family can share
Share a few details and we’ll follow up about availability, fit, and the right level of support.
Document literacy
We explain the purpose of common documents and help your family prepare information and questions for a qualified professional. That includes general education around powers of attorney, health care proxies, MOLST, wills, trusts, and insurance appeals.
Important boundary
SilverBridge provides educational, coordination, and advocacy services only. We do not provide legal advice, legal representation, or document preparation. For advice specific to your circumstances, consult a licensed New York attorney.
